We have an early stage, revenue producing chronic disease management business. Over the past 3 years, we have developed 2 licensed processes and platform systems that improve patient outcomes, deliver clinical pathways, collect all relevant patient and treatment data and if necessary, bill the 3rd party payors.

Obesity is a world wide problem that threatens the quality of life for millions of people around the world. Cost estimates associated with treating this problem are in the mega billions.

Our Pro Connect program helps Bariatric surgeons evaluate candidates for surgery, provides an Electronic Medical Record (EMR) for all patient data as well as treatment and outcome tracking.

Our CORE program provides Primary Care Physicians and/or groups that practice in a Federally Qualified Healthcare Center (FQHC) the capabilities to treat obese patients where it matters most - in the office. A recent study published in the American Medical Association Journal validated our approach to obesity treatment and management.

A recent CMS decision to provide payment for services related to the CORE program has created a time sensitive opportunity to scale our business.

The Company is searching for an investor or group of investors who are interested in helping us deliver our life changing programs to the 450,000 PCP's, 800 Bariatric surgeons and 1000's of community-based healthcare groups around the US.

Our Board of Medical and Industry experts are world class physicians, clinicians, scientists and business operators. We are engaged in developing an alliance with the Johns Hopkins School of Medicine. Our Pro Connect program was clinically reviewed by the University of Illinois Medical Center.

Our leadership team combines the talent and experience of business operators, senior level systems developers, marketing and sales executives and healthcare industry experts.

To scale our current business model in the obesity management space, we require an late seed/early growth investment of $2-3 million over the next 18-24 months.

In the future, we expect to migrate into other chronic disease categories that are co-morbidities associated with obesity such as diabetes, hypertension, wound care, vision loss and end of life care. We also see international opportunities as obesity is a global problem. Extensions into these categories and markets will generate additional revenues as well as may require additional capital.
